Sifilis adalah penyakit menular seksual yang disebabkan oleh Treponema pallidum.

PENYEBAB
Bakteri Treponema pallidum.

Bakteri ini masuk kedalam tubuh manusia melalui selaput lendir (misalnya di vagina atau mulut) atau melalui kulit.
Dalam beberapa jam, bakteri akan sampai ke kelenjar getah bening terdekat, kemudian menyebar ke seluruh tubuh melalui aliran darah.
Sifilis juga bisa menginfeksi janin selama dalam kandungan dan menyebabkan cacat bawaan.

Seseorang yang pernah terinfeksi oleh sifilis tidak akan menjadi kebal dan bisa terinfeksi kembali.

GEJALA
Gejala biasanya mulai timbul dalam waktu 1-13 minggu setelah terinfeksi; rata-rara 3-4 minggu.
Infeksi bisa menetap selama bertahun-tahun dan jarang menyebabkan kerusakan jantung, kerusakan otak maupun kematian.

Infeksi oleh Treponema pallidum berkembang melalui 4 tahapan:
  1. Fase Primer.
    Terbentuk luka atau ulkus yang tidak nyeri (cangker) pada tempat yang terinfeksi; yang tersering adalah pada penis, vulva atau vagina.
    Cangker juga bisa ditemukan di anus, rektum, bibir, lidah, tenggorokan, leher rahim, jari-jari tangan atau bagian tubuh lainnya.
    Biasanya penderita hanya memiliki1 ulkus, tetapi kadang-kadang terbentuk beberapa ulkus.

    Cangker berawal sebagai suatu daerah penonjolan kecil yang dengan segera akan berubah menjadi suatu ulkus (luka terbuka), tanpa disertai nyeri. Luka tersebut tidak mengeluarkan darah, tetapi jika digaruk akan mengeluarkan cairan jernih yang sangat menular.
    Kelenjar getah bening terdekat biasanya akan membesar, juga tanpa disertai nyeri.
    Luka tersebut hanya menyebabkan sedikit gejala sehingga seringkali tidak dihiraukan. Luka biasanya membaik dalam waktu 3-12 minggu dan sesudahnya penderita tampak sehat secara keseluruhan.

  2. Fase Sekunder.
    Fase sekunder biasanya dimulai dengan suatu ruam kulit, yang muncul dalam waktu 6-12 minggu setelah terinfeksi.
    Ruam ini bisa berlangsung hanya sebentar atau selama beberapa bulan. Meskipun tidak diobati, ruam ini akan menghilang. Tetapi beberapa minggu atau bulan kemudian akan muncul ruam yang baru.

    Pada fase sekunder sering ditemukan luka di mulut.
    Sekitar 50% penderita memiliki pembesaran kelenjar getah bening di seluruh tubuhnya dan sekitar 10% menderita peradangan mata. Peradangan mata biasanya tidak menimbulkan gejala, tetapi kadang terjadi pembengkakan saraf mata sehingga penglihatan menjadi kabur.
    Sekitar 10% penderita mengalami peradangan pada tulang dan sendi yang disertai nyeri.
    Peradangan ginjal bisa menyebabkan bocornya protein ke dalam air kemih.
    Peradangan hati bisa menyebabkan sakit kuning (jaundice).
    Sejumlah kecil penderita mengalami peradangan pada selaput otak (meningitis sifilitik akut), yang menyebabkan sakit kepala, kaku kuduk dan ketulian.

    Di daerah perbatasan kulit dan selaput lendir serta di daerah kulit yang lembab, bisa terbentuk daerah yang menonjol (kondiloma lata). Daerah ini sangat infeksius (menular) dan bisa kembali mendatar serta berubah menjadi pink kusam atau abu-abu.
    Rambut mengalami kerontokan dengan pola tertentu, sehingga pada kulit kepala tampak gambaran seperti digigit ngengat.
    Gejala lainnya adalah merasa tidak enak badan (malaise), kehilangan nafsu makan, mual, lelah, demam dan anemia.

  3. Fase Laten.
    Setelah penderita sembuh dari fase sekunder, penyakit akan memasuki fase laten dimana tidak nampak gejala sama sekali.
    Fase ini bisa berlangsung bertahun-tahun atau berpuluh-puluh tahun atau bahkan sepanjang hidup penderita.
    Pada awal fase laten kadang luka yang infeksius kembali muncul .

  4. Fase Tersier.
    Pada fase tersier penderita tidak lagi menularkan penyakitnya.
    Gejala bervariasi mulai ringan sampai sangat parah.
    Gejala ini terbagi menjadi 3 kelompok utama :
    - Sifilis tersier jinak.
    Pada saat ini jarang ditemukan.
    Benjolan yang disebut gumma muncul di berbagai organ; tumbuhnya perlahan, menyembuh secara bertahap dan meninggalkan jaringan parut. Benjolan ini bisa ditemukan di hampir semua bagian tubuh, tetapi yang paling sering adalah pada kaki dibawah lutut, batang tubuh bagian atas, wajah dan kulit kepala.
    Tulang juga bisa terkena, menyebabkan nyeri menusuk yang sangat dalam yang biasanya semakin memburuk di malam hari.
    - Sifilis kardiovaskuler.
    Biasanya muncul 10-25 tahun setelah infeksi awal.
    Bisa terjadi aneurisma aorta atau kebocoran katup aorta. Hal ini bisa menyebabkan nyeri dada, gagal jantung atau kematian.
    - Neurosifilis.
    Sifilis pada sistem saraf terjadi pada sekitar 5% penderita yang tidak diobati.
    3 jenis utama dari neurosifilis adalah neurosifilis meningovaskuler, neurosifilis paretik dan neurosifilis tabetik.

Neurosifilis meningovaskuler.
Merupakan suatu bentuk meningitis kronis.
Gejala yang terjadi tergantung kepada bagian yang terkena, apakah otak saja atau otak dengan medulla spinalis:
- Jika hanya otak yang terkena akan timbul sakit kepala, pusing, konsentrasi yang buruk, kelelahan dan kurang tenaga, sulit tidur, kaku kuduk, pandangan kabur, kelainan mental, kejang, pembengkakan saraf mata (papiledema), kelainan pupil, gangguan berbicara (afasia) dan kelumpuhan anggota gerak pada separuh badan.
- Jika menyerang otak dan medulla spinalis gejala berupa kesulitan dalam mengunyah, menelan dan berbicara; kelemahan dan penciutan otot bahu dan lengan; kelumpuhan disertai kejang otot (paralisa spastis); ketidakmampuan untuk mengosongkan kandung kemih dan peradangan sebagian dari medulla spinalis yang menyebabkan hilangnya pengendalian terhadap kandung kemih serta kelumpuhan mendadak yang terjadi ketika otot dalam keadaan kendur (paralisa flasid).

Neurosifilis paretik.
Juga disebut kelumpuhan menyeluruh pada orang gila.
Berawal secara bertahap sebagai perubahan perilaku pada usia 40-50 tahun. Secara perlahan mereka mulai mengalami demensia.
Gejalanya berupa kejang, kesulitan dalam berbicara, kelumpuhan separuh badan yang bersifat sementara, mudah tersinggung, kesulitan dalam berkonsentrasi, kehilangan ingatan, sakit kepala, sulit tidur, lelah, letargi, kemunduran dalam kebersihan diri dan kebiasaan berpakaian, perubahan suasana hati, lemah dan kurang tenaga, depresi, khayalan akan kebesaran dan penurunan persepsi.

Neurosifilis tabetik.
Disebut juga tabes dorsalis.
Merupakan suatu penyakit medulla spinalis yang progresif, yang timbul secara bertahap.
Gejala awalnya berupa nyeri menusuk yang sangat hebat pada tungkai yang hilang-timbul secara tidak teratur. Penderita berjalan dengan goyah, terutama dalam keadaan gelap dan berjalan dengan kedua tungkai yang terpisah jauh, kadang sambil mengentakkan kakinya.
Penderita tidak dapat merasa ketika kandung kemihnya penuh sehingga pengendalian terhadap kandung kemih hilang dan sering mengalami infeksi saluran kemih.
Bisa terjadi impotensi.
Bibir, lidah, tangan dan seluruh tubuh penderita gemetaran. Tulisan tangannya miring dan tidak terbaca.
Sebagian besar penderita berperawakan kurus dengan wajah yang memelas.
Mereka mengalami kejang disertai nyeri di berbagai bagian tubuh, terutama lambung. Kejang lambung bisa menyebabkan muntah. Kejang yang sama juga terjadi pada rektum, kandung kemih dan pita suara.
Rasa di kaki penderita berkurang, sehingga bisa terbentuk luka di telapak kakinya. Luka ini bisa menembus sangat dalam dan pada akhirnya sampai ke tulang di bawahnya.
Karena rasa nyeri sudah hilang, maka sendi penderita bisa mengalami cedera.

DIAGNOSA
Diagnosis ditegakkan berdasarkan gejala-gejalanya.
Diagnosis pasti ditegakkan berdasarkan hasil pemeriskaan laboratorium dan pemeriksaan fisik.

Ada 2 jenis pemeriksaan darah yang digunakan:
  1. Tes penyaringan : VDRL (venereal disease research laboratory) atau RPR (rapid plasma reagin).
    Tes penyaringan ini mudah dilakukan dan tidak mahal. Mungkin perlu dilakukan tes ulang karena pada beberapa minggu pertama sifilis primer hasilnya bisa negatif.
  2. Pemeriksaan antibodi terhadap bakteri penyebab sifilis.
    Pemeriksaan ini lebih akurat.
    Salah satu dari pemeriksaan ini adalah tes FTA-ABS (fluorescent treponemal antibody absorption), yang digunakan untuk memperkuat hasil tes penyaringan yang positif.
Pada fase primer atau sekunder, diagnosis sifilis ditegakkan berdasarkan hasil pemeriksaan mikroskopis terhadap cairan dari luka di kulit atau mulut. Bisa juga digunakan pemeriksaan antibodi pada contoh darah.

Untuk neurosifilis, dilakukan pungsi lumbal guna mendapatkan contoh cairan serebrospinal.

Pada fase tersier, diagnosis ditegakkan berdasarkan gejala dan hasil pemeriksan antibodi.

PENGOBATAN
Penderita sifilis fase primer atau sekunder bisa menularkan penyakitnya, karena itu penderita sebaiknya menghindari hubungan seksual sampai penderita dan mitra seksualnya telah selesai menjalani pengobatan.
Pada sifilis fase primer, semua mitra seksualnya dalam 3 bulan terakhir terancam tertular. Pada sifilis fase sekunder, semua mitra seksualnya dalam 1 tahun terakhir terancam tertular. Mereka harus menjalani tes penyaringan antibodi dan jika hasilnya positif, mereka perlu menjalani pengobatan.

Antibiotik terbaik untuk semua fase sifilis biasanya adalah suntikan penisilin:
- Untuk sifilis fase primer, suntikan diberikan melalui kedua bokong, masing-masing 1 kali.
- Untuk sifilis fase sekunder, biasanya diberikan suntikan tambahan dengan selang waktu 1 minggu.
Penisilin juga diberikan kepada penderita sifilis fase laten dan semua bentuk sifilis fase tersier, meskipun mungkin perlu diberikan lebih sering dan lebih lama.
Jika penderita alergi terhadap penisilin, bisa diberikan doksisiklin atau tetrasiklin per-oral selama 2-4 minggu.

Lebih dari 50% penderita sifilis stadium dini, terutama sifilis fase skunder, mengalami reaksi Jarisch-Herxheimer dalam waktu 2-12 jam setelah pengobatan pertama.
Reaksi ini diyakini merupakan akibat dari matinya jutaan bakteri.
Gejalanya adalah merasa tidak enak badan, demam, sakit kepala, berkeringat, menggigil dan semakin memburuknya luka sifilis yang bersifat sementara waktu.
Penderita neurosifilis kadang mengalami kejang atau kelumpuhan.

Setelah menjalani pengobatan, penderita sifilis fase laten atau fase tersier diperiksa secara teratur.
Hasil positif dari pemeriksaan antibodi biasanya menetap selama beberapa tahun, kadang seumur hidup penderita. Hal ini tidak menunjukkan adanya suatu infeksi baru. Untuk mengetahui adanya infeksi baru dilakukan pemeriksaan darah yang lain.


PROGNOSIS

Setelah menjalani pengobatan, prognosis untuk sifilis fase primer, sekunder dan fase laten adalah baik.
Prognosis untuk sifulis fase tersier pada hati atau otak adalah buruk, karena kerusakan yang telah terjadi biasanya tidak dapat diperbaiki.
